Botox®  vs. Xeomin®: Choosing the Right Neurotoxin to Combat Wrinkles

By FoRM Health | January 29, 2025

At FoRM Health in Portland, Oregon, we specialize in helping you look and feel your best—which may include rewinding back the clock on wrinkles using neurotoxin injections. If you’re considering injections to smooth those unwanted signs of aging, you’ve likely come across two popular options: Botox® and Xeomin®. While both yield wonderful results, they have some key differences that may influence which one is right for you. Let’s break it down.

What Do Botox™ and Xeomin™ Have in Common?

Both Botox® and Xeomin® are FDA-approved neurotoxins (derived from botulinum toxin) for the treatment of wrinkles of the forehead, frown lines (also known as “11” lines), and crow’s feet around the eyes. They work by temporarily relaxing the muscles that cause the dynamic wrinkle (wrinkle caused by movement). The result? Smoother, more youthful-looking skin. Both treatments are quick, minimally invasive, and require no downtime.

3 Key Differences Between Botox® and Xeomin®

Purity of Formulation

    • Botox®: Botox contains additional accessory proteins alongside the active ingredient, botulinum toxin. While these proteins are generally considered innocuous, they theoretically could trigger an immune response resulting in antibody formation against the proteins, potentially reducing its effectiveness over time.
    • Xeomin®: Known as a “naked” neurotoxin, Xeomin® is free of added proteins, containing only the active ingredient. This pure formulation may reduce the risk of developing antibody resistance, making it a great choice for long-term use.

Stronger vs. Lighter Effect:

    • Botox®: With the additional proteins, Botox® has a greater molecular weight than Xeomin®, resulting in Botox™ having a more precise and “heavier” effect in the treated area. This feature makes it the better option for individuals with strong facial expressions or those seeking minimal to no facial movement within the treated area.
    • Xeomin®: Without the accessory proteins, Xeomin™ has a more diffuse distribution within the treated area, resulting in a “lighter” land. This “lighter” effect is thought to create a more “natural” look, allowing for some muscle movement while turning down the wrinkle formation.

Onset + Duration of Results:

    • Botox®: With a slightly stronger effect, results are typically first noticed 3-5 days after treatment, though the full effect may take up to 2 weeks to settle in. On average, the results from one Botox® treatment lasts about 3 months.
    • Xeomin®: The lighter quality of Xeomin® may take a few more days to settle in, typically showing up 5-7 days after treatment, with the full expression of results showing up at 2 weeks, just like Botox®. Xeomin®, however, has been reported to last longer for some patients, averaging 3.5 month duration of results. The difference in duration of effect may be attributed to the accessory proteins.

Which One Is Right for You?

The best neurotoxin for you depends on your individual needs, lifestyle, and goals. Some patients prefer Xeomin® for its pure formulation and more natural aesthetic, while others stick with Botox® for its consistent and sometimes stronger results. During your consultation at FoRM Health, Dr. Kai will discuss your aesthetic goals and help you choose the option that’s best suited for you!

Tip #2: Exercise is more fun with friends! Plan to meet friends for a hike, group class or friendly competition!

🔹 In Your 30s? Cortisol imbalances from stress and poor sleep can disrupt hormonal cycles, cause fertility issues in both men + women, and set the stage for inflammation.

Tip #1: Establish a decompression routine you follow every night before bed.

Tip #2: Stress management should be simple; think breath-work, 5-Min journal, relaxing baths + sleepy-time tea!

✨ In Your 40s? Focus on strength training to support muscle + bone health while maintaining metabolic fitness through blood sugar control and stress management.

Tip #1: Train 4-6x per week with a balance of strength, mobility, and restorative movement (think yoga, Pilates). We highly recommend working with a qualified trainer to dial in your best form!

Tip #2: Swap steady-state, endurance cardio for 20 min of interval training (short bursts of speed + recovery work).

✨ 50s & Beyond? Hormonal changes in post-menopause & andropause can accelerate aging, affecting muscles, joints, brain, heart, skin, and more. Addressing imbalances can keep you vibrant and strong!

Tip #1: Talk to your doctor about bio-identical hormone therapy.

We all have a goal of best metabolic health, right We all have a goal of best metabolic health, right?! Well it’s time to work smarter not harder 💪 🧠 🔥

Sprinting + HIIT are powerful tools for metabolic health:

⚡️ Improve insulin sensitivity (your cells use glucose more efficiently)
⚡️ Stimulate mitochondrial repair (better energy production)
⚡️ Preserve & build lean muscle, which supports a higher resting metabolism (your basal metabolic rate)
⚡️ Create an “afterburn” effect (increased calorie burn after the workout)

Our nutritionist also uses sprints + high intensity interval training for conditioning in tennis, backpacking, & dancing! 🪩 🎾 🪩

If you’re just starting — this doesn’t mean going all-out on day one. Begin with short intervals, moderate effort, & plenty of recovery.

We also don’t need & shouldn’t do HIIT everyday!
2x per week is a sweet spot & you can get so much done in 10-20mins

& yes, longer forms of cardio like walking is super important for overall health too! Or long runs if you love to run but ROI for our metabolism really shines with HIIT. ⭐️

The goal isn’t maximum effort every day — it’s using intensity to our advantage!

Reframe weight loss in 2026. The goal isn’t a smal Reframe weight loss in 2026.
The goal isn’t a smaller number on the scale — it’s more muscle.

Building and preserving lean muscle is the foundation of long-term strength, metabolic health, and resilience. The real win? Improving body composition: less body fat, more muscle.

Here’s where to focus 👇
• Strength train (prioritize lower body): 2–4x/week
• Move more overall: aim for ~8,000 steps/day
• Fuel muscle growth: eat about ~1.5 g protein/kg of goal body weight daily

If you’re doing the work and still not seeing results, targeted supplements and (when appropriate) medications can provide additional support.

Here’s why Dr. Kai loves lower body strength-train Here’s why Dr. Kai loves lower body strength-training🤓

In women, muscle mass and strength begins a slow decline in our 30s, accelerates through menopause transition and—if ignored—can lead to frailty later in life.

The importance of muscle mass + maintaining strength extends beyond our musculoskeletal system, as lean muscle mass also plays critical roles in immune function, cardiovascular health, and blood sugar regulation. Less muscle = poorer glucose control, higher risk for diabetes, cognitive decline + heart disease.

Fighting the good fight to fend off frailty is best to start early (think 30’s, early 40’s) when hormones are still working for you, though it’s really never too late to start. Building muscle helps protect us from falls, injuries, vulnerability + eventual loss of independence.

This is why lower body training is extremely important. Your glutes, quads, and hamstrings are your largest + most metabolically active muscle groups (+ they’re the first muscles to weaken in menopause) — they protect your joints, stabilize your spine, maintain your metabolism + keep you resilient + protected in your own body.

Dr. Kai’s go-to lower body exercises 🏋️‍♀️

🔥 Deadlifts
🔥 Squats
🔥 Hip bridges / hip thrusts

These compound movements recruit multiple muscle groups at once (abs, back, upper body), build strength for the real-world, and deliver the biggest return on investment.

Tips for success 👇
Lift moderate to heavy to build muscle
Aim for muscle fatigue with 8-10 reps, 2-3 sets/ exercise
Train lower body at least twice/week

Plan for the future - Lean into lower body training today!

Muscle matters — at every decade of life. 💪✨
But a Muscle matters — at every decade of life. 💪✨
But as we age, it becomes non-negotiable.

Here’s why prioritizing muscle is one of the most powerful health decisions you can make:

🔹 Injury prevention + joint support
Strong muscles stabilize + protect joints, adds integrity to our physical framework, reduce injury risk + help with injury rehabilitation - muscle is key to resiliency + recovery!

🔹 Weight management + blood sugar control
Muscle is metabolically active tissue. Healthier muscle means better insulin function, steadier blood sugar, easier to manage weight, and meaningful protection against our most common chronic diseases — including heart disease, dementia, obesity, and diabetes.

That’s why improving body composition matters more than chasing the scale: the real goal is building muscle while reducing visceral (abdominal) fat.

🔹 Preventing rapid muscle loss (sarcopenia)
During the menopause transition, declining hormones (mostly estrogen, testosterone and DHEA) accelerate muscle loss. Maintaining and building lean muscle helps preserve strength, bone density + keep us from fading away.

At FoRM, we don’t chase quick fixes — we build resilient bodies that age well. Strength training, nutrition, hormone support, and recovery all matter.

Muscle isn’t just about looking good. It’s about choosing to thrive rather than survive!

Over the next few weeks, we’ll be taking a deeper dive into the many benefits of muscle. Stay-tuned 🎧
