Custom Combo Facial
A custom skin rejuvenation experience catered to your individual needs and goals β blending multiple therapies, this service addresses the underlying causes of aging such as inflammation, dehydration, and slowing cellular repair + renewal. During this session, youβll receive skin-specific exfoliation, facial massage with targeted serums and mask. Additionally, our advanced aesthetician will layer in a combination of hydrodermabrasion, microcurrent, dermaplaning, chemical peel, ultrasonic exfoliation, LED and/or radio frequency therapies.
Microneedling
Microneedling refers to a collagen induction technique using an automated medical device consisting of several tiny needles, which stimulates a healing response resulting in the regeneration of healthy skin cells with increased collagen (tightness) and elastin (elasticity). Microneedling is best for early signs of aging, acne, and texture issues.
Hydrodermabrasion
Hydrodermabrasion is a non-invasive exfoliation procedure used to renew overall tone and texture using a diamond tip and vacuum applicator, which polishes the outer layer of skin for a smoother, more even complexion. This exfoliation process is combined with the application of targeted toners and serums to treat skin specific concerns and reduce inflammation. Hydrodermabrasion is best-suited for patients seeking a glowing complexion, with improvements to texture, pore size, pigmentation and superficial scarring. It is also a great way to prep skin for more advanced procedures such as microneedling, IPL and laser resurfacing.
Chemical Peels
Chemical peels are a quick and effective cosmetic procedure using a topical acidic solution to achieve a smoother, more even complexion by removing the outermost layers of skin cells. The type of peel is chosen based on the specific skin concern to be treated, such as overall dullness, fine lines and wrinkles and minor sun damage. An advanced peel treatment is best for the client seeking to repair and rejuvenate the skin. Advanced peels require more downtime and a pre-treatment consultation is recommended.
LED Therapy Facial
Light therapy is a non-invasive system that is used to treat concerns such as pigmentation and sun damage, as well as signs of aging and loss of skin elasticity. Our comprehensive light therapy facial promotes collagen production, reduces inflammation, improves cell turnover and increases circulation to the skin to help you achieve a healthier complexion quickly, without pain or downtime.
Microcurrent
Microcurrent is a non-invasive method of facial toning that improves skin elasticity and fine lines. The application of electric stimulation via probes or gloves stimulates the production of ATP at the cellular level. ATP is the energy source of our cells, making microcurrent a great addition to any age-management skin care goals. This facial includes a custom serum application for intense hydration, lifting of the skin and overall skin integrity.
Dermaplaning
A surgical blade is used to remove dead skin cells and vellus hairs to create a smoother and brighter complexion. Fine lines and wrinkles are softened and pigmentation reduced with the added application of glycolic acid to prepare the skin for treatment and aid the exfoliation process. Dermaplaning promotes healthy cell turnover, even skin tone and texture and is well tolerated by many skin types.

If you are new to the Aesthetic Medicine program at FoRM, there are 2 ways to get started. One option is to start with a 15 minute complimentary consultation with our aesthetician who will review your goals and recommend a skin care regimen and services to help you meet your goals. Your first treatment will be scheduled on a different day. If you are looking to get started right away with a treatment, but donβt know where to start, you can schedule a 1 hour new client appointment with our aesthetician who will complete your consultation and perform an initial treatment on the same day. There is a $50 minimum charge for the one hour appointment, even if you decide against receiving a treatment that same day.
The cost of our custom combo facial ranges from $225 for a 60 minute treatment to $300 for a 90 minute facial. The custom combo facial combines 3-5 of the advanced aesthetic services listed above.
Facials are recommended monthly to every 3 months (seasonally) depending on skin conditions.
Hydrodermabrasion costs $125 per treatment. Packages of 3 are discounted by 10%, packages of 6 are discounted by 20%.

Advanced Chemical Peels cost $90. Packages of 3 are discounted by 10%, packages of 6 are discounted by 20%.
Improvements are appreciated after the first treatment. To optimize results 3 β 6 monthly treatments may be needed.
Peels are typically performed monthly until goals are reached, then seasonally (every 3 months) for maintenance.
Each microneedling treatment is $300. Packages of 3 are discounted by 10%, packages of 6 are discounted by 20%.
For best results, a series of 3-6 monthly treatments are recommended, depending on treatment goals. Some patients report immediate improvement in skin appearance, but most notice initial improvement 1 month after first treatment. Each treatment provides additive improvement to skin appearance.
While our office is located in SE Portland, Oregon, many of our patients travel from the greater Portland area, including: Lake Oswego, Beaverton, Hillsboro, Tigard, Milwaukie, Happy Valley, and Oregon City, for in-office visits and procedures. We also offer virtual office visits, which allows us to provide service to anyone seeking care within Oregon, with the exception of procedures which must be performed in office.
